These charges include regular diet and general nursing care of the patients and are subject to revision from time to time. There are some rooms with couch facilities for companions of the patients and, in such cases, an extra charge for Rs. 750 per day will be levied. Such arrangements, however, are subject to approval of the attending doctor. A room once taken with ‘couch’ will continue as such during the entire stay of the patient. Meals and other services will be charged extra for the companions staying in such rooms. No servants, companions, relatives, ayahs or children are permitted to sleep or remain any where in Clinic premises, save as above. However, in case one desires to stay as a companion in a 2, 3 or 4 bed room, we may consider the request if patient’s condition permits for which the charge shall be Rs. 450 on a daily basis. In case an additional pass for a single room is required, the charges for such additional pass shall be Rs. 600 per day. For day care cases, extra couch/companion charges are also same. There shall be no charge for mother staying with child upto 5 years age in any bed.

ROOM BOOKING

Reservation can be made by the Patient’s Doctor through the Admission Office or by the Patient / Relative with a Doctor’s prescription. Patients with infectious diseases, which may be dangerous for others will bot be admitted to the Clinic. If subsequent to admission any infectious disease is noticed, in the interest of other patients and employees we will expect the patient to take discharge to suitable hospital for such cases.

The Clinic only takes tentative booking of rooms and for the type of room. Room and type of Room to be alloted depends solely on availability at the time of admission. Patients shall be responsible to pay for the occupied/used room and thereafter of the room shifted to. In case a patient desires a single room and is unable to wait, he/she may take admission in ICCU / CCU / ITU and pay the relevant charge and on availability of single room shall shifted when suitable charge shall be levied. Direct admission to HDU unit is not allowed. Only transfer to HDU is allowed from ICCU / CCU / ITU / ICU. A patient, if transferred from ICCU / CCU / ITU / ICU to HDU, subsequently requires ventilator support in HDU, shall be considered as an ICCU / CCU / ITU / ICU bed and ICCU / CCU / ITU / ICU charges will be applicable.

TRANSFER/CHANGE OF ROOM

A patient of ICCU / CCU / ITU / HDU / ICU on recovery may be advised by Doctor for transfer to a normal room. Clinic shall arrange transfer to desired/class of room provided the same is available and till such transfer patient shall continue to occupy / pay for the room being used.Any kind of bed shifting will be done after 2 pm.

SPECIAL SECURITY

In case any patient is provided with exvlusive/special security, the same would be on chargeable basis.

PAYMENT OF BILLS

Following amounts required to be deposited at the time of admission or on the subsequent day by those not under cashless insurance coverage. The amount, however may vary depending upon the type of cases. All bills must be settled in cash, draft, pay order, electronic transfer, debit & credit card before leaving the clinic. Deposits should be made as per reminder given by floor sisters / staff / management.

Patients will be billed for one full day on the day of arrival. Normally patients are expected to take admission after 09.00 A.M.

In case they get admitted prior to 09.00A.M., their room will have to be booked from the previous day. They will, however, not be charged for the day they leave, provided they vacate the room before 12 Noon. No Lunch will be provided on the day of discharge. The cash counter in the first floor will be working from 9 A.M. to 5 P.M. Patients who wish to leave the Clinic before 9 A.M. on the day of their discharge should settle all their bills before 5 P.M. the previous day and obtain a clearance slip. Such patients should ensure that their attending Doctor writes the discharge well in advance on the previous day so that bill preparation is not delayed. All payments are to be made strictly in cash or by credit card. In case any patient desires to leave after 5PM, when cash counter in closed, they shall make a lump sum deposit with Matron’s office as suggested by the Sister on Duty and for final settlement the patient should come on next day with payment receipts for final settlement. All are requested to check their bills carefully before discharge or payment.

A card will also be given separately on admission showing above deposit figures. It is also displayed in Admission Office.

MATERNITY BOOKING DEPOSIT

For advance booking of maternity slots, a non refundable deposit of Rs. 4000/- has to be made which can only be adjusted against the final bill and settlement at the time  of discharge.

MEDICLAIM INSURANCE (CASHLESS ADMISSION)

For patients desirous of availing of Cashless facility under Mediclaim insurance policies, prior authorization for planned admissions is necessary. Such facility can also be availed of in emergency admissions subject to subsequent authorization by the insurance providers. For this, patient should fill up the applicable form and have the relevant columns filled up by their surgeon/ physician as the case may be. Although the Clinic will make efforts to obtain the cashless authorization, the responsibility remains with the patient. Patients should, therefore, try to have clearance from their doctor on previous day to expedite preparation of final bill & enhancement of sanctioned amount timely thus avoiding payment for an additional day. From clinic’s side full Co-operation shall be provided. Since insurance TPA does not sanction full estimate before discharge, final bill and discharge certificate have to be faxed for enhancement which will take time at the end of the insurance TPA for which clinic does not take responsibility and if the clearance comes beyond 12 Noon, one day’s extra room charge will be levied. Patient should settle the non-admissible items of the bill and any amount in excess of authorization, at the time of discharge. All reports in original are to be forwarded to the insurance provider by the clinic.

Management reserves the right to refuse cashless facility at any time for any one or all TPA’S. In case any short payment is received from
Insurance Co. or TPA against claim, Management reserves the right to claim the shortfall from the patient subsequently.
Before taking admission please ensure that we have a tie up with your insurer.

PRIVATE NURSES

The Clinic is staffed with fully trained and qualified nurses and it is our endeavour to give maximum attention to our patients. If, however, certain patients require private nurses, the Admission office may be requested who will try to arrange for such nurses from the Nurses Bureau. At the time of engaging the service of private nurses, preference will be given to serious cases and a decision in this regard will be taken by the clinic authorities. The charges of the private nurse is to be settled directly with the nurse concerned. Please note private nurses will be provided only if available. The clinic does not take/accept any responsibility for the services of the private nurses.

RESIDENT DOCTORS

Round the clock resident doctors are available who can be consulted for your patients’ condition at any time as per your convenience. Request for this is to be made through floor sisters. These Doctors will be attending your patients round the clock. In case Resident Doctors are to issue the discharge note or undertake any procedures, there shall be a nominal charge for these. These charges are subject to revision form time to time. WARD CONSUMABLES CHARGE

There shall be a levy of Rs. 250 as word Consumables Charge for each patient for stay upto 7 days. Thereafter it shall be again charged at each interval of / upto 7 days. This is charged to cover to some extent cost of consumables used in the floor for patient in general as it would be otherwise very costly to charge to full extent each item.

Sometimes the patients wish to get their meals from their homes. In such cases, the food should be brought only during the following timings and deposited with the Receptionist on the first floor who shall arrange to send it to the patient’s room.

Only veg diet is served in wing B. On Tuesdays and first half of Saturdays, veg food is served in wing A & C. Further Veg food will be provided to all PPN Insurance Patients and WBGH Scheme Patients admitted in any bed.

TELEPHONES

Telephone facility is available in the patient’s room, except at the ICCU, ITU, CCU, ICU & HDU. The Telephone connection will be withdrawn if it is advised by the attending doctor or the patient/relatives so desire. In case of Trunk Call, the same will be charged.

ADMISSION FEES & MRD CHARGE

For each admission, Rs. 700 is levied as registration charges and Rs. 750 for MRD Charges (Medical Record) as we keep / preserve records for 5 years under safe castody. For OPD cases Admission Fees is Rs. 100 and MRD charge is Rs. 100.

XEROX & FAX CHARGES

Our charges for these services are as under:

These charges are payable at actuals in cash counter and only after payment Xerox / Fax shall be done.

LAUNDRY

Patient’s personal clothes can be laundered at special rates. For details please contact the Sister-in-Charge of the floor.

PHARMACY – Medicines & Implants

The Clinic has a pharmacy to meet your requirements of medicines, baby food, toiletries, implants and cosmetics. All such requirements of patients shall be provided by the Clinic from the Pharmacy against prescriptions/advice of the treating Doctors. In case any medicine etc. is not available in our Pharmacy, we shall try to procure from other outlets within the City and, in case still not available, we shall request the relatives to procure the same and/or inform treating Doctor to suggest alternative/s.

AMBULANCE / HEARSE VAN

For 24 Hrs facility, you may contact Admission Office who shall arrange the same as per your requirement, charges for which shall have to be settled directly.

BLOOD BANK

Situated on 4th Floor of wing C, open for 24 hours everyday Voluntary (and Replacement) blood donation is welcome. For planned surgery and transfusion, please contact Blood Bank at least 48 Hrs before. In emergencies, the same is provided subject to availability of the particular group. Replacement donation for same at a later date will be highly appreciated. Blood can be supplied with/ without replacement within 1 hour of receiving the requisition and patient’s Blood samples from floor.

However, anyone opting / advised for the particular donated unit to be provided for his / her patients, the same should be donated at least 48 Hrs before to enable us to comply with the mandatory testing formalities. In dire situations, for the latter option, minimum 4-6 hrs should be allowed for processing the same before being made available to the patient for which an extra charge is levied.

Our Blood Bank is equipped to provide SAFE BLOOD to patients Tests for TTD (Transfusion Transmitted Diseases) are being carried out meticulously.

Procurement of Blood from outside Blood banks is discouraged. All blood requisition to be routed through blood bank.

To go to Blood Bank on 4th Floor, a special pass is to the collected from Reception Counter from 8AM-7PM and from Admission Office from 7PM-8AM. The pass is to be surrendered at Blood Bank.

OPD REGISTRATION

A levy of Rs. 150 per registration is made for OPD Doctor’s Consultation which is valid for one year. Consultant’s fees is to be paid separately. All prescribed tests should be done at the Clinic.

MORTUARY

This facility is available for maximum 24 hrs. at a Nominal Charge. We do not keep outside bodies. A charge is levied for linen used in such cases.

PATHOLOGICAL & OTHER TESTS

In case any special / particular tests cannot be conducted within the clinic, clinic shall make arrangement to do the tests from elsewhere and charge as per the bill of such outside agency with whom arrangements have been made. No other agency shall be entertained except those registered with the clinic.All biopsy tests are strictly to be done at the clinic.

BELLE VUE’S GARDEN CAFE- CAFETARIA

Beverages, cold drinks, drinking water and snacks are available between 8am and 8 pm at the cafe on the Ground Floor. It is a vegetarian cafetaria. Eating of any non-vegetarian items is prohibited at cafeteria

CORD CELL BANKING & NEWBORN GENETIC SCREENING

In case of maternity cases, the facilities of Umbilical Cord cell banking and Newborn Genetic Screening has been introduced for which details may be collected at Matron office at the time of booking. These facilities will be provided only through these organisation with whom clinic has made arrangements.

In order to afford maximum rest to the patient, pass system is prevalent as under :

  • Visitor’s Pass for Floors 3rd (Wing A&C), 4th, 5th (Wing A&B), 6th (Wing A&B) – 2 Passes.
  • Visitor’s Pass for Floors 2nd, 7th (ITU, HDU, ICU-1, ICU-2, ICCU, CCU, NU) – 1 Pass.
  • Night Stay Pass : 1 Pass.
  • Visitor’s Pass for availing couch facility : 1 Pass (Chargeable & Non Cancellable)
  • Visitor’s Pass for availing chair facility : 1 Pass (Chargeable)- Daily Basis.
  • Visitors are requested to please observe the visiting hours strictly which is essential for the proper care and comfort of the patients.

The liftman is authorized to inspect the pass before proceeding to the seventh floor for ICCU/CCU/NU. For ITU /HDU/ICU at second floor, one has to use the staircase and show the visitor’s pass to be Security Staff.

During visiting hours only one can go up the staircase. Beyond visiting hours one can go up only through the lifts.

For visit to Blood Bank on 4th Floor, visitor’s pass is required which is available from 8.00AM to 7.00PM from Reception and from 7.00PM to 8.00AM from Matron’s Office. Liftmen and Security Personnel are authorised to inspect the pass.

VALUABLES

Patients are requested not to bring or keep any money, jewellery or valuables with them during their stay in the Clinic. The Clinic does not take any responsibility for any loss of such valuables. Visitors to the patients are also requested to take care of their personal belongings. The Clinic does not take any responsibility for the thefts of any such items.

Foreign patients should deposit their passport with Matron office on admission and collect only on discharge.

MOBILE PHONES

Use of Mobile Phones – in common areas such as First floor Lobby, Corridors in floors etc. is forbidden. Similarly, in ICCU/CCU/ITU/ICU/NICU/HDU/SCBU, the use of mobile phone is forbidden. All are requested to switch off the mobile phones before entering such areas.

MUSICAL INSTRUMENTS

Electronic Players or other musical instruments may be played in the room with prior permission and at a volume which is not a cause of disturbance to others. After 10 P.M no such instruments will be allowed to be played. Television, wherever provided, should also be viewed at a low volume.

PARKING

Ample parking space is available for patient’s visitors. It is a paid parking which at present is Rs. 30 per hour for four wheelers and Rs. 20 per hour for two wheelers. Rates are subject to change any time.

24 HOURS SURVEILLANCE

Please note that you are under 24 hours electronic surveillance from the main road entrance in both Wings A & B and in all floors and every nook and corner of the complex.

SMOKING

Since Hospital is considered as a public place, smoking is forbidden under law and is treated as a punishable offence. Hence, all are to ensure that the law is not violated.

A spot fine of Rs. 300/- fine shall be levied which is payable in cash as per law for every violation.

LIFTS

Lifts should be used only while going up. Visitors are requested to please use the staircase while coming down. Visitors are also requested to give preference to patients and doctors while making use of the lift services.

BREAKDOWN

The management will not be responsible for failure of air conditioning system or any other technical/mechanical faults of any equipment. No claims what so ever, in such account will be entertained.

EXPANSION AND RENOVATION

From time to time these activities take place in the hospital. We regret the inconvenience from such activities and request for full cooperation. These are done for benefit of the patients and to improve services.

MANDIR-PRANAMI

Those desirous of giving pranami are requested to place their donation in box provided and not throw the same on the floor of the mandir or give to purohit/priest.

FLOWERS

Flowers are not permitted to be brought to the clinic for presenting to patients. One can present gifts as an alternative to flowers. Presentation of flowers has been stopped to avoid infection to patients.

GRATUITIES (TIPS) FORBIDDEN

Please do not tip the staff. Patients wishing to give gratuities are requested to place them in the box provided in each floor. Amounts so collected will be divided amongst all sub-staff equally.

The management will appreciate if they are immediately informed by the patients or their relatives in case any of our staff solicits tips in order to discourage this undesirable practice. Kindly note down their name carefully from their Identity Card and report to the Management or Ring 903996682.

SUGGESTION FORM

At the time of discharge, a suggestion form shall be given to the patient to be filled and returned duly sealed to the IPD Billing Counter in the 1st Floor. Till such suggestion form is received clearance shall not be given by the counter because it is very important that we receive these forms duly filled up as a feed back.

Please be free to write your suggestion and views. In case the suggestion form is not given due to over sight, the patients/relatives should ask for it and return the same duly filled.

SAFETY

The doors & windows of the rooms and bathrooms should always be kept closed to prevent hot air getting into the room which affects the air conditioning system of the clinic and also to keep the patients free from pollution. The clinic reserves the right without assigning any reason either to refuse admission to any prospective patients or to request any patient who has been admitted to leave the clinic. We shall be grateful for your co-operation and suggestions. Please help us to serve you better.

EVACUATION & FLOOR PLANS

We have displayed at strategic points our evacuation plan and floor plan. All are requested to study the same and follow it in case of any emergency. Please do not create any panic whatsoever.

COMPLAINT BOOK

This book is available with the Matron Office and will be made available on request. Please write your complaint and mention your contact details. Please mention patient’s name, bed/room no. and name of doctor under whom admitted.

When  you are examined you are entitled to privacy. If you are hospitalized , no outsiders may see you without your permission.

Your hospital records are private as well and no person or agency beyond those caring  for you may be permitted to see those records except the authorized statutory bodies.

2.Right to be treated respectfully:

You have the right to receive the best care medically indicated for your problem regardless of race, religion, national origin or the source of payment for your care. You have the right to prompt life saving treatment in an emergency without any discrimination on accounts of economic status or source of payment.

3.Right to privacy

you have the right to privacy . you may expect to talk with your doctors, nurse or our administration representative in private and know that the information you give will not be overheard or be given to others without your permission . In the hospital when you are in the semi private room you can expect a sincere and reasonable attempt to keep all conversations confidential.

4.Right to medical related information :

You have the right to seek and receive all the information necessary for you to understand your medical condition. You have the right to know the name of the doctor who is responsible for your care and to talk with that doctor and any other who give you care. You are entitled to know the probable diagnosis, planned course of treatment and the prognosis for the future. You have the right to ask your doctor or nurse any question that concerns you about your health.

5.Right for a second opinion

You have the right to select your doctor and your treatment. You have the right to say “yes” to the recommended treatment. You also have the right to say “No” to treatment and ask for medical consultation or a second opinion on your medical condition, if you so desire.

6.Right to leave the Hospital

You have the right to leave the hospital even if your doctor advises against it. If you decide to leave before the doctor’s advice, the hospital will not be responsible for any harm that this hat this may cause you and you will be asked to sign a “Discharge Against Medical Advice” Form.

7.Right to cost of treatment

You have the right to understand and receive information on the payment of hospital bills on a day-to-day basis in accordance with the policies of the hospital.

You and your designated relative have the right to factual information regarding the expected cost of treatment based on evidence. You will be given this information on request. You can see your detailed bill from the kiosk at reception. You will be informed about any additional cost to be incurred due to change in the physical condition of the patient or line of treatment. On completion of treatment,you have the right to receive a final bill, to receive an explanation for the bill(s) regardless of the source of payment or the mode of payment, and receive payment receipt(s) for any payment made.

8.Right to be free from any restraint or seclusion

You have the right to be free from any form of restraint or seclusion that is not medically necessary or used as means of coercion, discipline, convenience or retaliation by staff.
Restraint or seclusion may only be used to improve your well-being or protect you from harm and when you or your family members have been informed and given approval by written consent.

9.Right to informed consent

No medical treatment shall be administered until you or designated members from your family have consented to it. Such consent will be ‘informed’, based on the diagnosis, the nature of the proposed treatment, the risk involved (including pain and discomfort) and the chances and risks of alternative treatment of lack of any treatment at all. Your doctor will furnish the information to you at the earlist stage of the treatment in a manner that maximizes your ability to understand the information and to make a free and independent dicision.

10.Care under emergency

In case you are in a medical emergency, you will be eligible unconditionally to receive medical treatment. The doctor, to whom you turn up or being referred, is obliged to examine and treat you to the best of his/her ability. If the doctor is unable to do so he/she shall refer you to a facility that can provide the medical care which suits your requirements. In case of any life or limb saving situation, our doctor(s) will go ahead with the required emergency management, even against your will as per our hospital policy.

You, your family or your guardian has the right to tell us when something is wrong in the delivery of your care. If you do complain your care will not be affected in any way. Your feedback is welcome at all times.

Patients are entitled to receive information concerning your treatment including a copy of your medical record from your doctor or from the hospital. In cases, where such information may cause serious harm to your health or endanger your life, your doctor may decline to give you such information.
